Objective: To compare the characteristics of cesarean before and during JKN, also analyzes factors that influence these differences.
 Methods: This was a quantitative study with longitudinal retrospective design qualitative research used case studies. The population were delivery women at Banyuwangi referral hospital JKN. data analyzed firstly by univariable then bivariable. bivariable analysis performed comparing prevalence ratio (PR) between two variables.
 Results: proportion had increased significantly implementation 50% maternal deaths JKN gave birth using delivery, this to 60% Indications fraud committed health workers be factor in increasing delivery. Other contributing repeated delayed referral, number obstetric complications.
 Conclusion: There an increase deliveries BPJS Kesehatan needed re-evaluate system they created so far. quality service must emphasized negative impact on could minimized.

 Abstrak
 Tujuan: Membandingkan karakteristik persalinan sesar sebelum dan selama implementasi selain itu juga menganalisis faktor-faktor yang berpengaruh. Metode: Studi kuantitatif dengan desain studi kualitatif Populasi adalah ibu bersalin di rumah sakit rujukan Kabupaten Data dianalisis secara univariabel bivariabel. Analisis bivariabel membandingkan prevalensi rasio diantara dua variabel. Hasil: Proporsi meningkat signifikan meninggal metode sesar, proporsi ini menjadi 60%. Indikasi kecurangan oleh tenaga kesehatan salah satu faktor menyebabkan peningkatan ini. Faktor lain berkontribusi berulang, keterlambatan banyakanya komplikasi obstetrik. Kesimpulan: Terdapat perlu mengkaji ulang sistem dibuat Kualitas pelayanan harus tetap dipertahankan, agar tidak berdampak pada ibu. Kata kunci: jaminan kesehatan, kematian ibu, sesar.
